Kate
Here is the document I designed to help me keep track of my goals and projects for the month:
From Which Proverbs 14:1 Woman?

Yes, I know I'm neurotic.  But I love it!  =D  Everything is on one page.  For the two books I plan to read this month, there is a circle to color in for each chapter, plus one for typing my notes.

If you think I'm crazy, just smile politely and back away slowly.  If you think this is really nifty and would love to do something similar for your own goals, let me know.  If a Microsoft Word document will work for you, I can offer this as a customizable template for download.  If you're interested, you can either leave a comment or e-mail me.

Okay, I've got a "generic" Microsoft Word document put together; I'll post it late tonight. I know there must be a fancy-pants way instead of Microsoft Word, but I don't have time today to learn a new piece of software! I'll check with DangerBear and see what he thinks is the best way to do it. For now, I'll be offering the Word version. =)

You can check out my latest post (http://proverbs14-1.blogspot.com/2009/05/wfmw-neurotic-goal-planning-yay-free.html) for a downloadable version. =)

  8. Great idea! I love finding and designing forms, but my problem is that once they're done, I don't use them. Any advice on staying motivated enough to use them - or to even remember I have them? (I'm menopausal and I have been diagnosed with fibromyalgia, and I *will* blame them for problems remembering things - LOL!)

Susan, I also sometimes have trouble with following through. I'm great at making plans, not so great at "doing" them. I'm a very visual person, so I need to keep the form right in front of me. Right now I have it taped to the front of a folder that I carry around the house with me. You could also tape it to the refrigerator.

Something I like to do (but don't do it often because I think it would drive DangerBear nuts) is post little reminders all over the house. For example, if my goal is to floss my teeth each night, I'll write that on a Post-It note and stick it to the bathroom mirror.
